Will the NHS pay for treatment abroad? What the S2 route covers
Rules read 26 October 2026 (Taipei time)
Short answer
Only in a narrow case. NHS England's S2 route can fund planned treatment at a public provider in the EU, Switzerland, Norway, Iceland or Liechtenstein, if you apply and are approved before you go and the NHS confirms it cannot give you the treatment in a medically acceptable time (“undue delay”). The private clinics on this site, and every country outside Europe, are outside it (NHS: the S2 funding route).
- Covers
- Planned state (public) treatment in the EU, Switzerland, Norway, Iceland or Liechtenstein
- Never covers
- Private clinics, room upgrades, travel, hotels, translation
- When
- Approval before treatment; NHS England must confirm it cannot treat you in a medically acceptable time
- Decision
- Up to 20 working days for a complete application
The conditions, as NHS England lists them
- Prior approval: the application must be authorised before treatment.
- State healthcare: the treatment must be available under the other country's state scheme, not as a private patient.
- Available on the NHS: it must be treatment the NHS routinely provides to you in your circumstances.
- Undue delay: the NHS must confirm it cannot provide the same or equivalent treatment within a medically acceptable timeframe.
- Clinical support: written support from a clinician in that country after a full assessment, and the provider's planned dates and estimated costs.
- Residence: you must be ordinarily resident in England, registered with an NHS GP, and have seen the GP about the condition where relevant.
What you pay
If approved, you are treated on the same terms as residents of that country. Where residents pay a share (a co-payment), you pay it too and can claim it back on return; where state care is free, the S2 covers it all. You should not pay treatment costs up front, and an S2 cannot be approved for treatment already paid for. Travel and accommodation are never reimbursed.
An S2 is not a way to pay for the private clinics listed on our country pages. Those are self-pay. If you go privately, the NHS does not refund it.
How to apply
Download the S2 application form and guidance from the NHS page, collect the clinical support and the provider declaration, and email it to NHS England's European Cross Border Healthcare team. If refused, you can ask for a review with new evidence or appeal.
If S2 does not fit
Most readers who travel pay themselves. Compare the cost on our procedure pages: NHS MRI wait, NHS colonoscopy wait, NHS cataract wait, or start from the UK page.
NHS and treatment abroad: questions
Will the NHS pay for a private clinic abroad?
No. NHS England says the S2 route applies to public healthcare providers only and does not cover private treatment.
Will the NHS pay for treatment in Turkey, Thailand or Mexico?
Not through S2. The route covers planned state treatment in EU countries, Switzerland, Norway, Iceland and Liechtenstein.
Does S2 cover flights and hotels?
No. Only treatment costs are assessed. Travel, accommodation and translation are not covered, including for someone travelling with you.
How long does an S2 decision take?
NHS England says up to 20 working days for a complete application, longer if information is missing.
Can I pay first and claim back later?
No. An S2 cannot be approved if the treatment has already been paid for, except a co-payment that residents of that country also pay.
